For the 2022 season
Refund /Cancellation Policy: Any player who withdraws, for any reason after the season begins, from the association will be charged a cancellation/administration fee of $25.00. In addition, refunds will be pro-rated from the date of first floor time. As per SPTLA regulations, no refunds will be issued after May 1st. Registration Payment Options: Only credit cards /cash and/or EFT payments are accepted. Split Registration payments must be received by the dates required. No player is considered registered until payment is received in full. For insurance purposes, no player is allowed on the floor if payment has not been received. Registrations may still be accepted after March 2nd, space permitting.
Waiver I/We the Parent(s)/Guardian(s) of the above registrant, or the above registrant, hereby give my/our approval of his/her participation in all activities under the jurisdiction of the Alberta Lacrosse Association, its members and lacrosse clubs during the current year. I/We assume all risks and hazards to my/his/her participation in these activities as well as all risks and hazards incidental to the activities and transportation to and from the activities. I/We hereby release, absolve, indemnify and hold harmless all organizers, coaches, managers and officials appointed by the organizations and Associations mentioned above. I/We likewise release from responsibility any person transporting our child to and from the activities to the extent not covered by liability insurance. I/We will furnish proof of birth date of the above registrant upon request of duly authorized officials. Finally, I/We understand all of the above registration statements and agree to abide by all the rules and regulations as set forth by the Alberta Lacrosse Association. THIS CERTIFICATION MUST BE SIGNED BY A FULLY AUTHORIZED AND RESPONSIBLE PARENT OR GUARDIAN, IF UNDER 18 YEARS OF AGE REGULATION 15 - PRIVACY OF PERSONAL INFORMATION 15.01 Personal information will be collected to determine eligibility for competitive and recreational opportunities, age related events, to facilitate enrolment, to diseminate information, to communicate, to administer and evaluate programs and promotions that benefit Members, and for insurance and statistical purposes. In addition, personal information may be, from time to time, submitted to major funding bodies in order to verify registration and meet funding requirements. 15.02 All information must be collected with the consent of the person or legal guardian. 15.03 Personal information collection must be limited to what is absolutely necessary. 15.04 All efforts must be made to avoid incorrect information, and efforts must be made to verify the information. 15.05 Reasonable steps will be taken to protect the privacy of all personal information. 15.06 The Executive Director of the ALA will be the Privacy Officer for the organization. 15.07 Any member wanting access to their personal information may contact the Privacy Officer at 11759 Groat Rd Edmonton, AB T5M 3K6 or 780-422-0030 or ala_lax@telus.net.
